Medair is an international humanitarian organization established in 1988. Medair has been operating in Afghanistan since 1996 with programs mainly in the Health, Water & Sanitation, and Livelihoods sectors with significant reconstruction activities in remote areas. An Integrated Emergency Response: Medair aims to respond to high levels of food insecurity, malnutrition, and morbidity in Tirinkot district. The project intends to improve and extend WASH services including water supply infrastructure and hygiene practices to improve outcomes for vulnerable families.
An Integrated Emergency Response: Medair aims to respond to high levels of food insecurity, malnutrition, and morbidity in Uruzgan, which has been significantly affected by drought and conflict conditions. Building upon Medair’s decades of experience in the Central Highlands and Kandahar, the project intends to improve and extend the utilization of lifesaving preventive and curative nutrition and health services by mothers, PLWs, and children U5; to increase access to clean water, to reduce vulnerability to water-related and other preventable diseases, and to improve and increase food security for the most vulnerable families through food voucher distributions.
Job Description
Overall Purpose:
The position serves as the Senior Clinical Lead of the team, providing integrated health and nutrition services to targeted communities at the Basic Health Centre (BHC). The role includes delivering outpatient consultations for women and children and supporting BHC midwives and nurses to ensure the delivery of high‑quality care in line with standard guidelines and best practices.
The Senior Clinical Lead will oversee the implementation of key services, including Integrated Management of Neonatal and Childhood Illnesses (IMNCI), Reproductive Health (ANC, PNC, and Family Planning), as well as injury and trauma care and community health activities. The role also involves coordinating with the Expanded Programme on Immunization (EPI) team to improve immunization coverage within catchment areas and establishing effective referral pathways to strengthen linkages between project sites and other health facilities.
Overall, the position is responsible for leading clinical activities at the facility level and ensuring continuity of services. The female Medical Doctor (MD) will also assume leadership responsibilities and provide oversight in the absence of the supervisor.
RESPONSIBILITIES
- Provide outpatient consultations for women, and children under five, including the management of cases according to IMNCI guidelines. Provide clinical leadership by supporting IMNCI nurses and midwives to ensure the delivery of high‑quality care.
- Actively participate in clinical service delivery at the BHC, including the treatment of childhood illnesses, vaccination screening, Reproductive Health services (ANC, PNC, and Family Planning), as well as injury and trauma care, including first aid, initial treatment, and urgent referrals at assigned locations.
- Ensure that all staff at the Basic Health Centre (BHC) strictly follow national guidelines and organizational protocols in all service delivery areas.
- Coordinate with existing community health structures (Health Shura, CHNVs, CHWs, and other) within the assigned catchment areas, in accordance with local authority permissions.
- Ensure proper daily data collection and recording for all curative health activities, maintaining accuracy and completeness.
- Establish and maintain effective clinical referral pathways from each static site to appropriate higher-level health facilities.
- Oversee medicine and supply stock management at the clinic level, ensuring proper usage, storage, and timely submission of stock requests to the logistics team.
- Support overall service quality, ensuring continuity of care and patient safety at all times.
- Perform any other duties as assigned by the Line Manager and the Health & Nutrition Manager, in line with program needs.
MAIN ACTIVITIES
Supervisory responsibilities:
- Provide technical supervision and mentorship to BHC staff, including IMNCI nurses, midwives, and other clinical personnel, to ensure high‑quality service delivery.
- Ensure proper task allocation and duty scheduling among clinical staff to maintain efficient service delivery at all times.
- Conduct regular supportive supervision visits within the facility to ensure compliance with national standards and organizational protocols.
- Oversee adherence to infection prevention and control (IPC) measures and ensure a safe working environment for both patients and staff.
- Coordinate with the Health & Nutrition Project Manager and other relevant teams to address performance gaps and operational challenges.
- Support team communication and promote a collaborative working environment within the facility.
- Ensure that all staff maintain proper documentation, reporting, and accountability in their respective roles.
- Coordinate with staff and check patient flow and organize the team and the site efficiently and according to quality standards.
- Oversee the implementation of IMNCI, RH, injury and trauma care, and support community health activities, establish referral pathways to strengthen links between project locations and existing static health facilities in project areas of operation.
- Assist with on-the-job training of health team and nutrition staff in managing co-existing illnesses, triage, and thorough assessment and referral
- Ensure adherence to the organization’s values and professional behaviour of all nutrition staff and take the lead in resolving problems if needed.
- Support PSS staff with referral and follow up of cases requiring referral to Mental Health PHC consultation or specialised care.
- Prepare leave plan and follow up with staff to plan and take their annual leave one time.
- Ensure all relevant SOPs and other IEC materials are available at each department.
Technical responsibilities (involvement as needed):
- Provide medical assessment of children and women provide attending for PHC services and provide technical guidance and oversight to ensure all clinical services are delivered in accordance with national protocols and organizational guidelines.
- Support the implementation of IMNCI, Reproductive Health (ANC, PNC, and Family Planning), and other integrated health and nutrition services at the facility level.
- Ensure strict adherence to Infection Prevention and Control (IPC) standards and promote safe clinical practices within the health facility.
- Support accurate and timely data collection, reporting, and analysis, ensuring the proper use of standard tools and systems.
- Assist in identifying technical gaps and training needs and contribute to the capacity-building of clinical staff.
- Provide input into the planning and implementation of health and nutrition activities, ensuring alignment with project objectives and donor requirements.
- Support coordination with relevant stakeholders, including DoPH, EPI teams, and partner organizations, to strengthen integrated service delivery.
- Ensure proper use and maintenance of medical equipment and supplies and contribute to forecasting needs based on service demand.
- Develop health education plans and ensure their regular (daily) implementation and follow-up within the facility and community.
- Appropriate medical treatment following national protocols, IMNCI and RH guidelines.
- Examine children admitted to nutrition programme for co-existing illnesses/IMNCI or complications, and provide appropriate medical treatment or referral according to IMCI guidelines
- Review complicated IMNCI cases and especially children under 6 months and take the final decision on referral to the hospital
- Review complicated RH beneficiaries and take final decision on their referral.
- Ensure appropriate education and counselling is given to the beneficiaries and care takers
- Ensure treatment is given according to guidelines and protocols approved by National MoPH.
External representation:
- Liaise with stakeholders providing health in the same location, especially for referral
- In collaboration with the Health and Nutrition Manager and other BHCs operating in the same district ensure staff of referral facilities are informed of all aspects of the program including referral pathways
- Assist in mapping of villages and communities that have poor access to health and nutrition services
- In collaboration with the Health and Nutrition manager establish referral pathways to appropriate health facilities, paediatric and inpatient facility.
- Provide feedback to the health and nutrition Manager, on community specific challenges and barriers
Logistics:
- Prepare regular stock requests to ensure sufficient stock (medicine, medical supplies, and non-medical supplies) is available at each assigned BHC team location.
- Support and provide guidance to Pharmacy Assistant in safe storage of all medicine’s medical supplies and equipment.
- Monitor expiration dates to ensure stocks are used prior to their expiration date
- Report any suspected or identified fraud cases at the Health and Nutrition site to the Manager
Quality management:
- Ensure accurate record keeping of all activities at the static health facility.
- Submit required daily, weekly, and monthly reports. Carry out required quality checks according to provided checklists.
- Clinical supportive supervision and on the Job training to BHC team and community staff.
Job Requirements
Education:
- Minimum bachelor’s degree in medical Field is Required. Master’s degree preferred.
- Pediatrician is desirable.
- RMNCH, IMNCI, RH trainer is desirable.
Relevant experience:
- Minimum 5 years of experience is required as Medical Doctor.
- Experience of implementing BPHS/PHC/ IMNCI/ EPI programs is desirable.
- Working/implementing in BHC team is desirable.
- Working with an international organization is desirable.
Languages: Pashto oral and written skills is required. Good English oral and written skills is desirable.
Computer:
- Strong MS Excel, MS Word and ODK.
- Proficiency data analysis and reporting.
- Familiar with DoPH, PHC, and HMIS reporting.
Organizational capacity:
- Ability to organize a static health facility efficiently and orderly, coordinating team members and beneficiaries, identifying bottlenecks, and providing solutions.
- Ability to work with community leaders to arrange clinic sites, access, and security.
