Aga Khan Health Services (AKHS) is one of the most comprehensive private, not‐for‐profit health care systems in the developing world with community health programs in large geographical areas in Central and South Asia, as well as in East Africa.

The Aga Khan Health Services, Afghanistan (AKHS, A) was established in 2003 as part of the Aga Khan Development Network (AKDN). AKHS, A provides health care services mainly on behalf of the government including the Ministry of Public Health’s of Afghanistan’s Basic Package of Health Services (BPHS) and Essential Package of Hospital Services (EPHS). AKHS, A is unique both in scale and scope, delivering critical health services along the continuum of care, from Community Health Workers (CHWs) to ISO certified hospital facilities.

AKHS, A is currently implementing healthcare services in Bamyan, Baghlan, Badakhshan, Kabul and Takhar provinces. The agency’s scope of work includes the provision of primary and secondary healthcare, preventive and curative medical care, Nursing and Midwifery education, Human Resources Development and Hospital Management. As BPHS and EPHS implementer, AKHS, A provides technical support as well as capacity building of health personnel, immunization services, nutrition trainings and the training of Community Health Nurses and Community Health Midwives.

AKHS, A draws on internal and other donor fundings to continue supplementary and complementary support to the national health system. Moreover, AKHS, A has introduced innovations, including, expansions of e-Health services, e-learning initiatives, and cross border arrangements. To promote healthy environment, AKHS, A is also implementing net zero and greenhouse gas emission initiatives.

ثبت و انجام دادن ارزیابی نشوونما (GMP) که شامل اندازه گیری وزن، قد،سن وبررسی اذیما همه اطفال زیر دوسال

انجام مشوره دهی مبتنی بر GMP برای مادران / مراقبت کننده گان اطفال زیر دوسال در زمینه اقدامات مناسب تغذیه نوزادان، اطفال شیر خوار و مادران (MIYCN) و جلوگیری از انتان.

اگرطفل نارمل باشد، بمنظور تقویت و ارتقا روش های مناسب تغذی مادران و اطفال ، مادر تشویق و مشوره داده شود.

اگر طفل از سوتغذی حاد رنج می برد، برای تداوی مناسب و مشوره به داکتر مرکز صحی راجع شود.

برای مادران /مراقبت کننده گان مشوره داده شود که اطفال کمتر از دوسال خود را برای بازدید های ماهانه GMP به مرکز صحی بیاورند.

بررسی کنید که آیا طفل در جریان روز های ملی معافیت (NID) قبلی ویتامین A دریافت کرده است یا خیر در صورت عدم اخذ ویتامین A ، جهت اخذ ویتامین A به داکتر مرکز صحی راجع گردد

ارایه تعلیمات صحی به مادران در مراکز صحی به شکل روز مره.

برگزار کردن نمایش غذایی تکمیلی Food Demonstration)) ماه یک بار.

مشوره دهی در مورد تغذیه مادران

بر اساس یافته های MUAC و BMI (شاخص کتله بدن)، مشوره دهی انفرادی در مورد روش های مناسب تغذیه مادران ارایه گردد.

بررسی کنید آیا مادر شیر ده در جریان سه ماه بعد از ولادت ادویه اهن فولیک اسید IFA Tablet)) را به دست آورده است یا خیر.

اگر ظاهرا حالت صحی و تغذی مادر مناسب نبود برای تشخیص وتداوی به داکتر مرکز صحی راجع شود.

درسطح جامعه:

درجلسات ماهانه کارکنان صحی جامعه CHW)) با سوپروایزر صحی جامعه CHS)) و کارکنان صحی جامعه CHW)) اشتراک ، همکاری و هماهنگی داشته باشید جهت:

شریک نمودن لیست غایبین GMP یا CHW جهت  پیگری

حصول اطمینان از اینکه جلسه GMP توسط CHW به طور منظم در جامعه انجام میشود.

راهنمایی به CHW در موضوعاتی مانند GMP، مهارت های مشوره دهی، تعلیمات صحی و بازدید مناسب از منزل .

ثبت وگزارش :

گزارشات منظم برای نظارت و پیگیری از ارایه خدمات تغذیه داشته باشید .

استفاده از کتاب صحت مادر وطفل (MCH Hand Book) در مراکز صحی که این کتاب تطبیق میگردد.

مستفدین را که بازدید های بعدی نیاز دارند، شناسایی کرده و معلومات را با CHS  و CHW به اشتراک بگذارید .

 فارغ‌التحصیل صنف دوازده (۱۲) یا فارغ‌التحصیل مدارس دینی و در نبود فارغ صنف دوازده، حداقل صنف نهم (۹) نیز واجد شرایط خواهند بود، در صورتی که از محل باشند. همچنین، باید توانایی ارتباط به هر دو زبان ملی (پشتو و دری) یا زبان محلی را داشته باشند و اولویت به کسانی داده می شود که از همان محل باشند.
