Job Descriptions:

Specific Responsibilities:

 

  • Ensure effective implementation of community based health care programs at the provincial level.
  • Regularly supervise and monitor performances of CHWs at the HFs and health posts levels.
  • Establish community health committees and take part in community health committee meetings on monthly basis.
  • Identify health problem/s in community, share with the community and office, and make suggestions for solving the problem/s.
  • Responsible for supplying and re supplying of CHWs kit to HPs as per the requirements.
  • Collect MAAR report from all HFs, analyze the data and make all MAAR are entered into DHIS2 on regular monthly basis.
  • To identify the drop out of HPs, and report to the line manage on due.
  • Conducting CHW initial and refresher training according the project work plan.
  • Keep the provincial CHW profile updated (as monthly basis).
  • Practicing of HRCHW database and update it on quarterly basis and submit the updated database to deputy technical project manager on time.
  • Ensure HFs community maps are updated on annual basis.
  • Active participation in CBHC related meeting and improve coordination with CBHC department of DoPH.
  • Active participation in CHW national day ceremony at provincial and community levels.
  • Analyzing the referred cases between HPs and HFs.
  • Responsible to GPS all HPs within 1st quarter of project.
  • Strength filing system and archiving all CHW training documents (attendance sheets, per-diem sheets, drug supply, hygiene materials supply, HFs and HP community health council meetings minute) in SO and HFs levels.
  • Take active part in the community mapping, surveys and NIDs programs.
  • Support provision of primary health care within the community.
  • Perform limited, selected administrative duties such as patient charting, recording and reporting of data, clinic and/or facility management, or coordination of specific educational programs.
  •  Follow established departmental policies, procedures and objectives; continues quality improvement initiative; safety, environmental and infection prevention standards.
  • Identify and report immediately any problem which interferes in delivery of services or in achieving the program objectives.
  • Participate in weekly meeting of health facility to share information and discuss the program progress.
  • Perform as task assigned by the line supervisor.

Job Requirements:

  • Excellent written and oral communication skills in local languages
  •  Excellent interpersonal skills: demonstrated ability to interact professionally with culturally diverse staff.
  •   Experience in training and supervision of BPHS components
