Job Description

Afghanistan continues to face low routine immunization coverage, particularly among zero-dose and under-immunized children in polio high-risk districts. Nangarhar remain vulnerable due to population mobility, misinformation, weak follow-up of defaulters, and limited community-facility linkages. This project leverages Community Health Workers (CHWs) and Community Health Supervisors (CHSs) under the CBHC framework to increase routine immunization uptake, reduce zero-dose children, and strengthen maternal, child health, nutrition, WASH, and birth registration services in alignment with MoPH, Polio Emergency Plan, and UNICEF strategies. The field supervisor will provide regular supportive supervision, mentoring, and performance monitoring of CHS and CHWs to ensure effective community engagement, support outreach sessions, accurate tracking of zero-dose and defaulter children, and timely referral and follow-up of RI, MCH and nutrition cases.

Job Requirements

Key Roles and Responsibilities

  • Conduct regular field supervision visits to CHS and CHWs using standardized supervision tools.
  • Provide on-the-job coaching to improve CHW performance and data quality.
  • Facilitate monthly CHW coordination and review meetings at health facility level.
  • Support microplanning using child registration books and coordination with polio program DCOs, CCS and social mobilizers.
  • Verify CHW records on household visits, referrals, zero-dose tracking, and outreach support.
  • Submit regular supervision reports and flag gaps to the Project Manager.
  • Ensure timely referral and follow-up for ANC, PNC, institutional delivery, nutrition, birth registration, and RI services.
  • Promote adherence to PSEA and child safeguarding standards.
  • Support engagement with community leaders, women’s groups, and religious leaders.

Key Deliverables

  • Monthly supervision reports covering assigned CHWs.
  • Documented CHW coordination meetings and action points.
  • Verified referral and follow-up records.
  • Improved CHW performance and data completeness.

Qualifications and Experience Field Supervisors

  • Graduate of a recognized medical or health-related field and social science.
  • Minimum 2 years of experience in community-based health programs, preferably in immunization, Polio, maternal and child health, nutrition, or primary health care.
  • Experience in supervision, mentoring, or field-level coordination of FLWs or volunteers is an asset.
  • Skills in data recording, reporting, and use of simple digital tools.
  • Ability to conduct fieldwork and travel regularly within assigned districts.
  • Knowledge of CBHC structures and familiarity with local community dynamics is an advantage.
