Job Summary
The Outreach Vaccinator/CHS is responsible for administering vaccines in accordance with the national immunization schedule to prevent vaccine-preventable diseases. Ensures vaccine supplies are available and kept in proper temperature, maintaining accurate immunization records, conducting community outreach and awareness sessions, and supporting both routine and campaign-based immunization activities, provides vaccination services considering humanitarian principles (Humanity, impartiality, Neutrality and independency), provides immunization services of targeted group (women and children). The Outreach vaccinator/CHS is also responsible for overseeing and supporting community-based health activities within the assigned project area, ensuring the effective implementation of health promotion and disease prevention activities through the supervision and guidance of Community Health Workers (CHWs). The role includes capacity building of CHWs, monitoring of community-based interventions, data collection and reporting, and maintaining close coordination with health facilities and relevant stakeholders to improve community access to essential health and nutrition services. The CHS is accountable for planning, implementing, and reporting all Community-Based Health Care (CBHC) activities in accordance with the project scope of work and Ministry of Public Health (MoPH) policies, while ensuring strong coordination between local communities and health facilities for responsive, efficient, and quality service delivery.

The Outreach Vaccinator:
• Maintain a clean, safe, and child-friendly vaccination area while ensuring confidentiality and respectful treatment of beneficiaries.
• Receive vaccines, cold chain commodities, IEC materials and reporting formats/registers/cards from REMT/PEMT on regular basis as per need/BHC catchment area population and target groups.
• Conduct vaccine administration sessions in BHC catchment area for all target mothers and children on regular basis.
• Make sure that accuracy and quality of all field immunization activities are maintained and reported (EPI registers, clients’ cards & other reporting formats) on due time to REMT/PEMT and CARE sub office.
• Responsible to know the updated national EPI guidelines/policies regarding immunization program.
• Regular reporting to PEMT/REMT regarding vaccine, cold chain commodities and data including proper storage and inventory of all items received and utilized.
• Maintain specific focus on quality of vaccines through proper follow up of EPI recommendations for cold chain in the field.
• Prepare and maintain the immunization micro plan according to BHC fixed and outreach plan to reach all eligible mothers and children on due time.
• Organize vaccination session according to the BHC fixed and outreach plan for all catchment area population.
• Organize/conduct regular health education sessions during field work and pass health messages regarding immunization to clients and communities.
• Maintain close coordination with local community elders/stakeholders and volunteers to ensure motivation and mobilization of the local communities to participate in EPI activities.
• Registration and reporting of all vaccinated eligible mothers and children according to National EPI reporting formats/registers.
• Support vaccination campaigns, emergency immunization activities, and outbreak response when required.
• Keep record of all EPI related assets and cold chain commodities supplied by National EPI/WHO.
• Actively participation in community-based immunization awareness raising program.
• Undertake any other task assigned by the health facility in-charge.
The Community Health Supervisor (CHS).
• is responsible for overseeing and supporting community health activities within assigned project areas.
• The CHS ensures the effective implementation of health promotion, disease prevention, and outreach services through the supervision of Community Health Workers (CHWs).
• The role includes capacity building, monitoring community-based health care (CBHC) interventions, data collection and reporting, and coordination with health facilities and key stakeholders to improve community access to essential health and nutrition services.
• The Community Health Supervisor is overall responsible for planning, executing, and reporting all CBHC activities in line with the project scope of work and the Ministry of Public Health (MoPH) policies.
• The CHS maintains close coordination between local communities and health facilities, ensuring that project activities are implemented effectively and efficiently, contributing to the achievement of project goals and objectives.
• Assist HF staff in providing plans of community health programs in the health facility and the catchments areas.
• Conduct community awareness sessions as per Health Facility plan and IEC materials given by Health Facility In-charge/doctor on regular basis.
• Conduct regular health education sessions according to the health facility health education plan and organize public awareness sessions in target communities and HP level with active involvement of CHWs.
• Regular reporting to health facility regarding all CBC activities and CHWs monthly performance.
• To use and report Health committee information to monitor the health status and deliver the report to head of health facility.
• Coordinate with members of the basic health centre (BHC), and the Provision of Essential Life-Saving Healthcare Services to catchment are population as per CBC policy and project scope of work.
• Coordination Established between Health Facility and community in the surrounding areas of the Health Facility Local community awareness level improved regarding the health care services available in the health facility.
• Community level referral of increased to Health Facility.
• Health promoted in the target areas of the health facility.
Personal Skills:
Possession of a certificate of completion for the vaccination course.
