La Chaîne de l’Espoir is an international Non-Governmental Organization (NGO) founded in 1994. Its mission is to strengthen health systems to give everyone, especially children, the same chances of survival and development. The NGO has been working in Afghanistan since the early 2000s. In 2005, it built the French Medical Institute for Children (FMIC) in Kabul, a non-profit paediatric hospital, to provide high quality medical and surgical care for young patients. Since 2006, the Aga Khan Development Network (AKDN) has been responsible for the overall management of the FMIC, with La Chaîne de l'Espoir providing medical support and access to the most vulnerable populations. In 2016, the hospital expanded with the opening of gynaecology, obstetrics and neonatology services, becoming the French Medical Institute for Mother and Child (FMIC). Services for adults have also been developed in recent years.
The goal of La Chaîne de l’Espoir is to increase access to healthcare in particular for the most vulnerable Afghans. To do so, La Chaîne de l’Espoir:
• Trains and supports the medical, paramedical and technical teams through short- and long-term volunteer missions.
• Provides free surgical treatment at FMIC and social care for the most vulnerable children and women coming from all the 34 provinces of Afghanistan and offers patients and their caretakers accommodation and food in the Women and Children House (WCH) for free. The house has segregated common areas for men and women, private and shared rooms, a garden and a playroom.
1)TASKS & RESPONSIBILITIES:
General Description:
CDE, with financial support from the Private Hospital Foundation (FHP) in France, is launching a comprehensive project aimed at supporting reproductive, maternal, and newborn care in three government-run health facilities in the urban areas of Kabul province, Afghanistan. This initiative serves as a pilot project for CDE and will focus on enhancing the RMNCH services at Shiwaki and Buth Khak Comprehensive Health Centre (CHC) as well as providing night shift services at the Gosfand Dara Sub Health Centre (SHC) managed by PUAMI.
Specifically, CDE will oversee the night shift operations of the RMNCH unit at Gosfand Dara SHC, while PUAMI will handle the day shift. The project, scheduled to commence on January 1, 2025, and run until December 30, 2025, In the result of a collaborative effort between CDE, the Kabul provincial public health directorate, health facilities were selected.
2) MAIN RESPONSIBILITIES:
Midwives play a vital role in providing comprehensive support and care to women and their families throughout the entire process of childbirth and pregnancy. From prenatal care to postnatal care, midwives offer guidance, advice, and assistance tailored to meet the unique needs and preferences of each individual. They empower women to make informed decisions about their care and services, promoting autonomy and personalized care.
In addition to attending to the mother's needs, midwives also provide essential care for newborns, offering vital support and information on infant care and feeding. They serve as educators, equipping expectant mothers with the knowledge and tools necessary to maintain a healthy pregnancy and a smooth transition into parenthood. Furthermore, midwives offer ongoing parenting support to help families navigate the challenges and joys of raising a child.
One of the key responsibilities of midwives is the maintenance of the health and well-being of both the mother and the baby. Through their expertise and experience, midwives are equipped to identify and manage various aspects of pregnancy and childbirth, intervening only when necessary and referring to obstetricians in cases of complications or high-risk situations. This collaborative approach ensures that women receive the appropriate level of care and support throughout their childbirth journey.
• Cultivate and maintain a harmonious and supportive work environment among colleagues to enhance teamwork and cooperation.
• Ensure continuous functionality of health facilities by offering around-the-clock services, CDE staff specifically allocated to cover night shifts at the Gosfand Dara SHC (Sub Health Centre).
• Uphold the importance of providing respectful maternity care to patients to ensure their comfort and well-being.
• Emphasize the importance of punctuality for both day and night shifts to maintain operational efficiency.
• Actively engage in the morning report to ensure effective communication and exchange of vital information among staff.
• Prioritize comprehensive end-of-shift reports by midwives to ensure seamless transitions and continuity of care for patients.
• Require night shift midwives to remain on duty until the end of the morning report to facilitate the exchange of critical information.
• Maintain presence on duty until the relieving shift arrives and is fully briefed to ensure continuity of care.
• Uphold standards of accuracy and thoroughness in maintaining medical records for patients, ensuring they are neatly organized and readily accessible.
• Keep the patient board updated with relevant information to support efficient patient management and care coordination.
• Admit new patients promptly, conduct thorough labor assessments, complete necessary documentation, and order appropriate lab screenings in line with MoPH and BPHS guidelines.
• Identify high-risk pregnancies and emergency cases, initiate resuscitation measures, and promptly arrange referrals to linked hospitals.
• Supervise and assist mothers during labor, monitor new-born well-being, provide necessary support and pain management following advanced midwifery care protocols.
• Monitor labor progression, conduct necessary assessments, and promptly identify and address complications while documenting relevant records.
• Perform normal vaginal deliveries with adherence to established protocols, including post-delivery procedures and documentation.
• Maintain detailed records of deliveries in the Birth Register Book and ensure cleanliness and sterility of delivery instruments.
• Handle complex vaginal deliveries, such as vacuum extraction and shoulder dystocia, during labor’s second stage with competence and precision.
• Provide comprehensive newborn care, including assessment, routine procedures, and health education for parents during the postpartum period.
• Educate patients and families on various health topics, including hygiene, nutrition, family planning, warning signs, and postnatal care.
• Regularly check and replenish supplies and medications, adhering to protocols and promptly reporting deficiencies.
• Strictly adhere to infection prevention measures in all aspects of maternity care delivery.
• Foster a culture of knowledge sharing and skill development among maternity staff, promoting family involvement in the birthing process.
• Provide feedback to leadership, report incidents promptly, and actively engage in provided training opportunities to enhance skills and knowledge.
1) TEAM RELATIONSHIP
As midwife is a key member of team so must have good relationship with all team like gynecologist, vaccinator, nutrition councilor and psychosocial councilor
2) PROFILE
Professional Training:
• Registered Midwife, bachelor’s degree is preferred
• Certified midwife and attested by the MoPH
• Good communication skill both in writing and oral in Dari/Pashtu is essential
• Communication skill in English oral and written is desirable and is preferred
• Ability to communicate clearly and work well with others in a team environment
• Familiarity with the BPHS guideline and maternity care protocol
3) WORKING EXPERIENCE:
• Minimum 3 years’ consecutive work experience in the position of midwife is required
• Worked as a midwife in secondary level health facilities
• Worked in conflict areas
4) SPECIFIC SKILLS:
• Able to work on ultrasound and handle reporting
• Can work during night shift
• Can work alone in the clinic
5) LANGUAGE
• Excellent Pashtu and Dari
• Proficient in English
