Job Description

Job Summary:

The Community Health Supervisor (CHS) is responsible for overseeing and supporting community-based health activities within the assigned project area, ensuring the effective implementation of health promotion and disease prevention activities through the supervision and guidance of Community Health Workers (CHWs). The role includes capacity building of CHWs, monitoring of community-based interventions, data collection and reporting, and maintaining close coordination with health facilities and relevant stakeholders to improve community access to essential health and nutrition services. The CHS is accountable for planning, implementing, and reporting all Community-Based Health Care (CBHC) activities in accordance with the project scope of work and Ministry of Public Health (MoPH) policies, while ensuring strong coordination between local communities and health facilities for responsive, efficient, and quality service delivery. The CHS plays a key role in achieving project objectives by ensuring effective implementation of CBHC activities, strengthening community–facility linkages, and contributing to improved health outcomes at the community level.

Clinical / Facility-Based Responsibilities:

  • Support the Health Facility (HF) In-Charge in coordinating community-related activities linked to facility services.
  • Participate in HF and Health Post (HP)-level coordination meetings, including monthly health committee meetings.
  • Support the HF In-Charge in establishing and strengthening community health shuras at HF and HP levels, ensuring active participation of community elders and stakeholders.
  • Develop and implement schedules for monthly health shura meetings and ensure regular and effective engagement of community representatives.
  • Support the health facility in organizing service delivery, including patient flow management, screening/triage, and efficient utilization of services.
  • Maintain effective coordination and communication between the health facility and communities to improve service delivery and trust.
  • Assist HF staff in developing and implementing community health program plans within the facility and its catchment area.
  • Identify community health needs and ensure they are communicated and addressed through facility-based planning and services.
  • Conduct community awareness and health education sessions in line with HF plans and approved IEC materials.
  • Share community feedback, health trends, and CHW reports with HF staff to improve service quality and responsiveness.
  • Coordinate with Basic Health Centre (BHC) staff to support the delivery of essential health services in line with CBHC policy and project scope.
  • Ensure timely and accurate reporting to the Health Facility In-Charge and Field Supervisor on all CBHC-related activities and progress.
  • Deliver regular health education sessions according to the facility’s health education plan,
  • Responsible for supporting the community in finding and solving health problems.

Assist HF staff in providing plans of community health programs in the health facility and the catchments areas.

Community-Based Responsibilities:

Establish and strengthen community health shuras at Health Facility (HF) and Health Post (HP) levels, ensuring active participation of community elders, women, and key stakeholders.

·       Conduct regular field visits to communities, Health Posts, and Community Health Workers (CHWs) to monitor community-based activities and provide guidance.

  • Organize and facilitate capacity-building training for CHWs, and provide supportive supervision across all HPs within the HF catchment area, with emphasis on on-the-job training, community feedback, and reporting to the HF.

·       Prepare and implement schedules for regular community health shura meetings and ensure active participation and community ownership.

·       Maintain strong communication and coordination between communities, CHWs, and the health facility.

·       Support communities in identifying, prioritizing, and addressing key health problems through participatory approaches.

·       Mobilize communities for health campaigns, awareness sessions, and outreach services, ensuring inclusion of vulnerable groups.

·       Conduct regular community awareness and health education sessions using approved IEC materials.

·       Organize community-level public awareness events in collaboration with CHWs and community leaders.

·       Collect community feedback on health services and share findings with the health facility for improvement.

·       Ensure timely and accurate reporting of all CBHC activities, including CHW performance and community engagement.

·       Utilize community data (from CHWs and shuras) to monitor health status and support planning.

Job Requirements

Education:

 Graduated from 12 grade at least, and higher education level will be preferred especially medical institute.

Experience:

Languages Skills:

 Fluency in spoken and written Dari and Pashto is a requirement. English Language skills is a plus point.

 Report to Health facility In-charge.

Previous working experience (at least one year in the communities mainly involved in health education,

awareness, community mobilization and other social work).

Excellent problem-solving skill and decision making

Strong communication skills

Patient counseling and advice

Good teamwork spirit

Willingness to learn new skills.
